Between sessions, keep the conversation connected.
A simple way to organise notes, appointments and questions for your rehabilitation team.

The time between appointments can bring new questions about your programme. Keeping them in one place helps you make the most of the next conversation.
Know your contact options
Ask your provider which channel to use for routine questions, appointment changes and concerns about your assigned programme. An app message and a clinical appointment may serve different purposes. Clarify response times and which matters should not wait for a routine reply.
Keep useful notes
Record questions as they occur rather than relying on memory. Include enough context for the team to understand what you mean, such as the session or instruction you are asking about. Follow the team’s advice on which observations to track.
Keep the plan clear
If an instruction is uncertain, ask for clarification instead of making your own changes. Make sure you know which version of your plan is current and when the next review is scheduled.
Bring support into the loop, if you want
You can ask how a family member or caregiver may participate in updates. Agree on what information is shared and what role that person will have. Your clinician remains the right person to guide changes to your rehabilitation programme.
Pass it on to someone who might enjoy it.
