Your first assessment: make space for a conversation.
Organise your questions and practical details before meeting your care team.

An assessment is a chance for you and your care team to understand the next step together. A little preparation can help you communicate what matters to you.
Write down your questions
Keep a short list of the things you want to understand: how the assessment works, who will guide your programme, how follow-ups are arranged and how to contact the team. Bring any information the provider has specifically asked you to share.
Describe everyday priorities
Explain the activities or situations you would like to discuss. Use your own words and be specific about your concerns. You do not need to arrive with a diagnosis or a proposed exercise plan; those decisions belong in the appropriate clinical conversation.
Check the practical setup
Confirm whether the appointment is online, at home or at a clinic. For an online appointment, check the connection, sound and privacy of your space. Ask in advance whether another person should be present or whether any particular setup is needed.
Leave with clear next steps
Before the appointment ends, ask what happens next, what you should record and whom to contact with questions. Request clarification when something is unclear. This guide covers appointment preparation; exercises, restrictions and treatment decisions must be individualised by your clinician.
Pass it on to someone who might enjoy it.
